Cancellation Policy

Please know we aim to serve you and all of our clients with equal respect and consideration of your time. Please give advance notice if you need to cancel or reschedule an appointment.

We require a minimum of 48 hours notice to cancel a consult or follow-up appointment. Otherwise a 50% of the appointment fee will be charged.

We require your credit card information before we can assess your intake form. Your card will not be charged until your consultation appointment is booked.

If you are not able to attend your initial consultation once your intake form is submitted please call the office. There is a $50 administrative fee to process your intake form. You will be reimbursed the remainder of funds less the $50 intake form administrative fee.

If you are calling about testosterone replacement therapy, we only take men 40 years of age and over. No exceptions.
